If you have already done a complete rescan more than once, then I'm out of suggestions. The rescan seems to have cleared up the problem for everyone else.
As I said in the other thread, we cause the problem by changing the PID on our video data for Time Warner Cable.  But that was almost two months ago.

In looking around some more, the over the air + DirecTV version of the same receiver (the T165) has a similar problem on stations that actually transmit their program guide.
I believe TMJ is still the only Milwaukee station sending out the program guide as part of the PSIP data.
Samsung solved the problem for the T165 model by upgrading the T165 receiver firmware via DirecTV.

Here's what I found about the "PSIP lock problem" and the T165 upgrade on the AVS Forum:

"PSIP Lockup" from changing channels from Directv to OTA or back – if you select an OTA channel that updates their PSIP (guide and information banner) regularly it will cause the unit to lose Video and Audio when the information doesn't match up from the last you used the channel – you can still get Guide and Menu selections to show on the screen, but that's it. To get the picture and sound back you have to reboot the unit. This can be done in two ways – unplug the unit and plug it back in – it will retrieve the guide again and after a short period the sound and picture will come back on. The other way is to reboot from the front panel buttons (not the remote). Push the "Menu" and "Right >" buttons at the same time. This will bring a Diagnostic Menu on screen. Cursor down to the Command Line, press "OK" and one of your selection will be "Reboot", press OK, press OK again and the new Guide will load up and picture and sound will come back. Until Samsung and Directv download a software fix for this, (expected soon -- current software version is 1.0.04) you'll have to live with it or stay away from the OTA stations that say anything except "Regular Schedule" at the top of the information banner (this means they are not sending out PSIP). RESOLVED - THE 1.0.06 FIRMWARE DOWNLOAD CORRECTED THIS!

I have not seen anything about a firmware upgrade for the T160.

Have you tried Samsung technical support: 1800-SAMSUNG or email techsup@seasvc.samsung.com

just to reduce confusion, the box which got the satellite software upgrade was the TS-160.  The TS-165 is the over-the-air "only" box.

The AVS forum has a thread which I started which has become a good source of information on the TS-160.  And yes, Samsung had sent an engineer to my house from Korea after my discussions with the senior development manager for Directv HD and SD receivers.  It was an unbelieveable level of support from Samsung.  They solved the problem in just a few hours but it took 2 months for it to be sent via Directv due to quality control issues.
