Getting Cable Back

You can call me a hypocrite because in previous posts I probably said that I am never getting cable back, but a recent move, while still in the Milwaukee area, has no roof antenna, and rabbit ears do not really work, pixilation crazy.  I am going to order digital cable in the near future.  My question is I'll ask for the cable card, but of course I will also need a tuning adaptor.  I cannot remember will they be able to give me the tuning adaptor with a request over the phone or do I have to fill out a form on there website to request one.
