Can anyone tell me if the $9.99 charge would apply to each TV?

Nope.  Just a monthly charge.  There is a mirroring fee of 4.99, but from what I can tell, it's only one charge for multiple receivers.

Correct, there is only one HD fee, the mirror fee is also called the lease fee now.
